normal style=text-indent: 24pt; margin-right: -28.15pt;>1、項目內容及范圍：同心路（氣象局門口至洛書大橋段）、興寧路（民族路至迎賓大道）、永寧路（長虹路至高速收費站）、福寧大道（長虹路至同心路）、鳳翼路（民族路至同心路，含鳳翼路北）等5條主干道綠化看管養(yǎng)護。

3、服務期限（養(yǎng)護期）：3年

六、供應商資格要求：

normal style=line-height: 21pt; text-indent: 24pt;>合格供應商除符合《政府采購法》第22條外，還必須符合下列條件：

normal style=line-height: 21pt; text-indent: 24pt;>1、供應商應具有獨立法人資格，持有《營業(yè)執(zhí)照》，營業(yè)執(zhí)照的經營范圍必須包含園林綠化等服務內容。

normal style=line-height: 21pt; text-indent: 24pt;>2、項目負責人應具備相應的現場管理工作經歷和相關專業(yè)中級以上（含）工程師證書。

normal style=line-height: 21pt; text-indent: 24pt;>3、根據《財政部關于在政府采購活動中查詢及使用信用記錄有關問題的通知》（財庫〔2016〕125號）和《河南省財政廳關于轉發(fā)財政部關于在政府采購活動中查詢及使用信用記錄有關問題的通知》（財庫〔2016〕15號）被列入中國政府采購網(www.ccgp.gov.cn)“政府采購嚴重違法失信行為記錄名單”的，被列入中國執(zhí)行信息公開網（http://shixin.court.gov.cn/）“失信被執(zhí)行人”的、被列入國家稅務總局網站（www.chinatax.gov.cn/）——重大案件查詢欄目“重大稅收違法案件當事人名單”的供應商將被拒絕參加投標。供應商必須將本公司在上述三個網站相關欄目的信用記錄截圖打印，分別裝訂入投標文件正副本（應當將信用記錄清晰展開打印，否則視同未提供）。

normal style=line-height: 21pt; text-indent: 24pt;>6、供應商的被委托人、項目負責人應為本單位職工，提供2017年11月以來連續(xù)一年社保繳納證明（以社保中心網頁截圖或社保中心出具的社保證明為準，加蓋供應商公章）；
